(hereinafter known as “Foster”) represent
that no member of, or individual living in the household where animals will be fostered whether temporary or permanent has been charged with or convicted of animal cruelty, neglect, or abandonment. By signing this agreement, I agree and adhere to the following terms and conditions below.
I understand that I am responsible for the wellbeing of the foster animal(s) temporarily in my care, and I am subject to all ordinances, rules and regulations of the city municipality, town, township, village, or county and all the laws of the state wherein the foster home and foster animal(s) reside. I understand and agree to adhere to all relevant zoning and animal control codes and ordinances, whether local, county or state.
I understand that there is risk associated with bringing a foster animal into my home, and I understand that Furever Donny Rescue cannot be responsible for medical treatment of individuals or personal pets living at home if they were to get sick or injured. In addition, I understand that Furever Donny Rescue recommends careful introduction to all pets and all personal pets to be up to date on vaccines.
I understand that my foster animal(s) may not be trained, and I may have to work with Furever Donny Rescue staff or designated trainer. This includes but is not limited to chewing, basic manners, anxiety, house, or litter training. This may result in the foster animal(s) having accidents in my home or causing damage. I understand that Furever Donny Rescue is not responsible for any damage that may be caused by these behaviors.
I agree to provide the foster animal(s) with plenty of love and daily exercise, fresh food, fresh water and to incorporate the foster animal into my daily routine. I agree to take caution with feeding and treating and to feed my foster animal(s) separately from other pets living in the household.
I understand and acknowledge that I do not have any right or authority to give, adopt, transfer, or place foster animals in other homes or with other individuals or organizations, unless approved by Furever Donny Rescue staff.
I understand that Furever Donny Rescue is the sole and rightful owner of the animal (s) in foster care. I agree that every animal I provide foster care for must be physically returned to Furever Donny Rescue at any time upon request. I also agree to return the foster animal(s) immediately if I am no longer able to provide adequate care.

I understand that I will receive information on any foster animal(s) I take in that will disclose all treatments received while in the care of FDR. This disclosure shall include immunizations, medications, and any other veterinary treatments administered prior to transfer. As the recipient of the foster animal (s) I will sign off on this disclosure and agree to continue and properly document any medications/ treatments that have been in place as prescribed.
I agree to always keep foster dog(s) within a fenced yard (not on a chain) or supervised on a leash. I agree to never leave foster dog(s) outside unattended and to never allow the foster dog to be tied outside on a chain. I agree that feline foster(s) in my care will always be kept indoors.
I understand that I must consult with a member of Furever Donny Rescue staff before taking a foster animal(s) to any veterinary practice and all veterinary care must be authorized in advance by Furever Donny Rescue. I understand that if I choose to take my foster animal(s) to a veterinary practice on my own accord, it will be completely at my own expense, and I will not be reimbursed by Furever Donny Rescue.
I understand that Furever Donny Rescue will not offer reimbursements for expenses incurred during the fostering period. FDR will directly cover approved veterinary expenses. We ask that fosters cover the cost of food for their foster animal(s) if possible. All foster homes will be provided with items such as crates/kennels, bedding, toys, clothing dependent upon the need.
I understand that if any foster animal(s) is harmed due to my negligence, I will be held responsible for the veterinary bills, surgery cost and/or medications.
I agree to always transport foster animal(s) safely and never to allow them to be loose in the open bed of a pickup truck or in any other unsafe manner.
I agree to notify Furever Donny Rescue immediately of any upcoming changes of address, telephone number or any change in living situation or environment.
I acknowledge that Furever Donny Rescue staff or volunteers of Furever Donny Rescue have not made any warranties regarding the health, temperament, breed or conduct of my foster animal(s) I agree and understand that animals can be unpredictable, and that Furever Donny Rescue cannot insure against unexpected conduct of an animal in foster care.
I agree to hold Furever Donny Rescue harmless from any direct or consequential damages arising out of this Foster Agreement. I hereby accept to foster animal(s) for Furever Donny Rescue and assume all the risks and responsibility associated with their animals; including but not limited to scratches, bites or injury. I fully and completely release, indemnify, and hold harmless Furever Donny Rescue, it’s staff, Board of Directors, volunteers, and agents from any claim, cause or action or liability, whether, known or unknown, directly or indirectly arising out of or in connection with their
I understand in accordance with state regulations, all foster care homes are required to have an annual inspection. I agree to have a Furever Donny Rescue representative conduct an inspection of my home annually or as deemed necessary.
I acknowledge that Furever Donny Rescue may terminate this or any other arrangement with me at any time at its sole discretion.
By signing and submitting this form, I certify that I am at least 18 years of age and have read this document fully. I understood and agreed to all the above terms and conditions. I have had the opportunity to show this agreement to an attorney or voluntarily choose to sign without doing so.
