(certifying at least 18years of age) fully understand and agree to assume all risks involved in all duties that I perform for Furever Donny Rescue, also known as FDR, in my capacity as a volunteer or foster for this organization.

 

In consideration of my participation as a foster care provider or volunteer, I hereby release and discharge, indemnify, defend and hold harmless, the Board of Directors of Furever Donny Rescue, their officers, employees, agents, volunteers, members and individuals and entities contributing support, from any liability for damage to loss of personal property, sickness, injury from whatever source, legal entanglements, imprisonment, death, loss of funds, etc. which may occur as a result of my participation, I agree that my participation is strictly voluntary. I agree to foster and/or handle animals at my own risk, and to be solely responsible for any damage to people or property caused by the animal(s).  I am fully aware of, understand and agree to all the terms and conditions of this release and indemnification provision, and that it is binding and enforceable by law.

 

I give FDR permission to take and use any photograph(s) video(s) or audio(s) of me and/ or my foster animal(s) or personal pets for educational, marketing, and promotional purposes of Furever Donny Rescue including but not limited to webpages, newsletters, brochures, and event displays. I further understand that I will not be paid for the use of such photograph(s) video(s) or audio.

 

If I will be sheltering or providing foster care or boarding for any Furever Donny Rescue’s animals

in my home I consent to Furever Donny Rescue visiting my home from time to time

to observe the animals and their living quarters.

 

 I have read and fully understand the terms and conditions of this agreement and acknowledge that Furever Donny Rescue may terminate this agreement or any other arrangement with me at any time in its sole discretion.

This Liability Release will forever prevent me from seeking any legal action whatsoever against Furever Donny Rescue organization, volunteers, or any of its representatives.
