The following agreement is between the adopter
and Furever Donny Rescue (FDR).
Please read carefully, as signing the document indicates that you understand and agree to comply with each item listed.
I agree to provide the adopted dog with good, quality daily care, including fresh food and water daily. I will provide routine health care, including but not limited to vaccines, parasite prevention, and regular veterinary check-ups.
I understand that there is no guarantee of the dog I am adopting’s health, age, breed, or temperament. I hereby accept the animal as is, assume all risks and responsibilities associated with dog ownership, and release FDR from any and all claims of liability,
I agree to register the dog’s microchip and update the database with any changes to my personal information. This is an added measure to ensure that if the dog is lost, it can be returned.
I agree not to have the dog crated for long hours or left unattended, tied, or chained up outdoors as a means of securing it. Proper fencing will be installed, maintained, and always used. The dog will also sleep indoors at night.
I acknowledge that any known medical conditions were made known to me prior to adoption (if applicable). I understand that the dog has received initial vaccinations and that additional vaccinations may be required, and I agree to provide these at my own expense. I understand I am responsible for all veterinary costs associated with this dog after adoption.
If, for whatever reason, I cannot keep the dog, I agree to inform FDR immediately, and FDR reserves the right to reclaim and reacquire the dog. I will not abandon, give, sell, or transfer the dog to another person, business, shelter, organization, or entity unless otherwise agreed upon by a FDR representative.
I am aware that a FDR representative may conduct a home visit at any time to ensure the welfare of the adopted dog and that all adoption policies are met.
I understand that after I take the dog home, there is a trial period of 7 days; if it doesn’t work out with this dog, I can return it within this time frame for a refund of the adoption fee, less a $50 non-refundable processing fee.
FDR retains the right to repossess the dog at any time if there is evidence of neglect, abuse, or if FDR has good reason to believe that the home is not in the best interest of the animal.
In consideration of my acceptance of assuming ownership of this animal from FDR, I hereby release and discharge, indemnify, defend, and hold harmless FDR, its staff, Board of Directors, volunteers, members and individuals, and entities contributing support from any liability for damage to or loss of personal property, sickness, injury from whatever sources, legal entanglements, imprisonment, death, or loss of funds which may occur as a result of my adoption. I have read and fully understand the terms and conditions of this agreement. I further understand that these provisions are binding and enforceable by law.
