Do you need a dog that is good with cats? In general what experiences do you have with pets? Have you owned a pet before? Please list all animals, living or deceased, you have owned in the last 10 years and what happened to them.
Have you or your partner ever given an animal to a shelter or had to re-home? What type and brand of food will you feed your dog?
How often will you feed your dog? Will you exercise your dog? How would you react if your dog destroyed a household object or soiled in the house?

Acknowledgements Furever Donny Rescue requires you to have a collar and name tag before taking possession of a dog.
understand that this application is used as part of the adoption approval process in order to determine the best home for the dog. The completion of this application does not guarantee the adoption.
I understand that FureverDonny Rescue will contact me within 72 business hours if I am approved and I will not be notified if the dog is already adopted or my home does not fit the dog's behaviors and personality.
I am eighteen(18) years of age or older. I will be required to present my State ID at the time of consultation.
I understand that Furever Donnyl Rescue does not guarantee nor claim to know the previous history, breed compilation, or temperament of my adopted animal. I understand that a rescue animal's breed may not be accurate. I release any further liability from Furever Donny Rescue regarding the size, temperament or breed of my adopted animal.
I understand that Furever Donny Rescue requires proof of home ownership or landlord approval documentation. If applicable, I am aware Furever Donny Rescue will be contacting my landlord to verify any pet restrictions including but not limited to age,breed, or size. I have completed any additional paperwork or made arrangements for deposits required by my landlord to be paid prior to adoption completion.
I understand that if I have other animals, Furever Donny Rescue requires verification of their current vaccinations. If applicable, I am aware that Furever Donny Rescue will be contacting my veterinary office to verify these vaccinations.
I understand that Furever Donny Rescue reserves the right to refuse adoption to any applicant
I certify that the above information in the application is true to the best of my knowledge. I also understand that giving false information on this application is grounds for denying an adoption.
As part of my adoption package, I have received the Colorado Department of Agriculture rabies prevention brochure as well as all medical and acquisition records that disclose treatments received before and during the care of FDR.
By providing my electronic signature, I verify that I am indeed said applicant, and that I have answered all of the questions truthfully to the best of my ability. I understand that if any information is falsified Furever Donny Rescue may confiscate the adopted animal and I will be responsible for any and all court fees.
